Registration Requirements

  • See our PPSD Registration Checklist below to ensure you have all the necessary documents to register for Providence Public Schools. 

    • Student Age Verification Document: Original birth certificate, passport, residency card, OR I-94
    • Parent/Guardian photo identification
    • Immunization Records
    • Physical Exam form
    • Proof of Providence Residency: Provide one of the following: Utility bill (not a cell phone or internet bill), pay stub, lease, bank statement-all dated within 60 days of registration
    • School records:  Most recent report card (Grades K-8) or Official High School transcript (Grades 9-12)
    • Students with special education services: All current Special Education paperwork 
    • Tuberculosis Screening Test: Upon enrollment in school, an incoming student is required to have a Tuberculin Skin Test (PPD) or TB
      blood test (IGRA) if any of the following indications are met:
      1. Birth/residence in, or travel for at least 90 days to any country outside of the contiguous US, Canada, Australia, New Zealand or a Western/Northern European country
      2. Close contact to someone with infectious TB disease during lifetime
      3. Immunosuppression (current or planned): HIV infection, organ transplant recipient; treated with TNF-alpha antagonist (e.g., infliximab, etanercept, others), or immunosuppressive interleukin antagonists, steroids (equivalent of prednisone ≥15mg/day for ≥1 month) or other immunosuppressive medication

    PARENT/GUARDIANS OF STUDENTS BORN OR LIVING OUTSIDE THE CONTINENTAL US OVER 90 DAYS

    We strongly encourage you to speak to one of our nurses (x13111 OR x13112) about your immunization requirements prior to initiating the registration process.  

    NOTE:  ALL PPD TESTING AND RESULTS MUST BE DONE WITHIN THE CONTINENTAL US.

  • AGE FOR ENROLLMENT

    Every child who is five years of age on or before September 1st of each school year shall be eligible to attend kindergarten. There are no exceptions to this policy.

     

    SPECIAL NOTEIf you are registering a student and you are not the parent or legal guardian, you must have a notarized document from the parent or legal guardian granting you permission to register the child.
