REGISTRATION CHECKLIST |

| ALL NEW STUDENTS | ||
| (INCLUDING THOSE WHO PREVIOUSLY ATTENDED PPSD AND LEFT FOR ANY PERIOD OF TIME) | ||
| Welcome! Providence Public Schools looks forward to welcoming your child to the classroom. Registration involves a few simple steps. | ||
| Please note that registration requires the student’s parent or guardian to visit the Student Registration & Placement Center in person (the child does not need to be present). | ||
| 1. Download and complete registration forms: | ||
| 2. Gather required documents (original or certified copy): | ||
| – Photo identification AND one: | ||
| – Student birth certificate identifying same parent or | ||
| – Document establishing guardianship or | ||
| – Notarized letter from a parent on the birth certificate identifying the guardian | ||
| – State issued Birth Certificate or Passport (no copies) OR | ||
| – Naturalization papers or I-94 Arrival card or Permanent Resident Card | ||
| – Changes with regards to Puerto Rican Birth Certificates | ||
| – Lease or mortgage statement; Section 8 agreement; gas, electric or phone bill; paystub or bank statement or | ||
| – If above not available: Notarized Affidavit of Residential Address – available here or at the Student Registration Center – AND proof of residency in the name of the person with whom you reside and is listed on the affidavit | ||

| – Grades 9–12: School transcript(s) with courses and grades | ||
| – Copy of IEP (Individualized Education Program) | ||
| 3. Explore your school options: | ||
| – Identify your neighborhood school(s) by calling the Student Registration & Placement Center (401) 456-9297 | ||
| – Use the PPSD website to view the School Profiles for information about location and school hours, programs offered, and assessment results for the schools you are interested in your child attending, as well as individual school web pages for more information. | ||
| – Schedule individual school visits | ||
| 4. Visit the Student Registration & Placement Center in person: | ||
| – Check-in at front desk and present registration forms and required documents. If the registration forms have not yet been completed, copies will be provided to be completed in the waiting area. | ||
| – Parents are called in order of arrival to meet with a placement officer to review the registration forms; the placement officer will take copies of required documents. | ||
| 5. If registration is complete, and a language assessment is not required, the parent/guardian will be asked for school preference for the child. | ||
| If the registration is complete and the home language survey indicates that a language assessment is required, an appointment will be made for the child to meet with the ELL teacher for assessment. After assessment and determination of educational program, the parent/guardian will be asked for school preference for the child. | ||
| 6. School placement is made after receipt of all required forms and documents. | ||
| Depending on the time of year, notification may be made by letter or phone call. Your Registration Center Placement Officer will advise you at the time of registration. | ||
